If you are a medical graduate preparing for post-graduation, there is no way that you’re not torn between two major exams: INI-CET and NEET PG.
Even though both the exams are a way to getting into the most esteemed postgraduate programs in India, there is a significant difference when it comes to their structure, difficulty level, and the institutions they lead to.
Now that the exam is drawing near, scheduled for 17th May, 2025, you need to be thoroughly clear about how the two exams compare. And one of the most important things to consider is how you can tailor your preparation to succeed in INI-CET.
In this blog, we’ll be talking in detail about all these differences. We are also going to discuss how PrepLadder Version X can give you a decisive edge in your final stretch of preparation.
INI-CET (Institute of National Importance Combined Entrance Test) is a centralized entrance exam conducted by the All India Institute of Medical Sciences (AIIMS), New Delhi.
It was introduced in 2020 to unify the PG medical entrance process for India’s most prestigious medical institutions. These include AIIMS (Delhi and other branches), JIPMER Puducherry, PGIMER Chandigarh, and NIMHANS Bengaluru.
The exam is held twice a year — once in May and once in November — allowing aspirants two opportunities annually to secure admission to highly sought-after MD, MS, DM (6 years), MCh (6 years), and MDS courses.
With only a limited number of seats and an extremely competitive selection process, INI-CET is known for its high difficulty level, analytical question style, and rigorous evaluation criteria.
What makes INI-CET especially challenging is its emphasis on conceptual clarity, clinical application, and the ability to handle twisted MCQs and image-based questions that mimic real-life diagnostic scenarios.
The exam has 200 multiple-choice questions and lasts 3 hours. It uses a +1 mark for correct answers and -1/3 for wrong ones.
NEET PG (National Eligibility cum Entrance Test – Postgraduate) is conducted by the National Board of Examinations (NBE).
It is the single-window entrance exam for admission into MD/MS/PG Diploma courses across government and private medical colleges in India, excluding the Institutes of National Importance which are covered under INI-CET.
Unlike INI-CET, NEET PG is held only once a year — usually in March-June. It covers a broader array of colleges and thus has a significantly larger pool of candidates competing for seats.
NEET PG also consists of 200 multiple-choice questions, but the exam duration is 3.5 hours, and the marking scheme is +4 for correct responses and -1 for incorrect ones.
The exam includes a mix of factual recall questions, clinical vignettes, and image-based questions, with an overall moderate to high difficulty level. It places slightly less emphasis on deep conceptual integration and more on breadth of coverage and speed. Because of this, NEET PG preparation often focuses on covering a wide syllabus efficiently.
While both exams are competitive and demand rigorous preparation, they differ in meaningful ways:
Feature | INI-CET | NEET PG |
Conducting Body | AIIMS | NBE |
Frequency | Twice a year (May & Nov) | Once a year |
Institutes Covered | AIIMS, JIPMER, PGI, NIMHANS | All other government/private medical colleges |
Mode of Exam | CBT (Online) | CBT (Online) |
No. of Questions | 200 | 200 |
Duration | 3 hours | 3.5 hours |
Marking Scheme | +1 for correct, -1/3 for incorrect | +4 for correct, -1 for incorrect |
Question Types | Clinical vignettes, image-based, conceptual | Clinical + factual + image-based |
Difficulty Level | Higher (more conceptual + analytical) | Moderate to high (clinical + factual balance) |
Understanding these differences can help you make a smarter decision — or even better, adapt your preparation to succeed in both.
Your choice between INI-CET and NEET PG largely depends on your career goals and target institutions. If your dream is to earn a seat at an Institute of National Importance, then INI-CET should be your primary focus. The prestige, faculty, infrastructure, and exposure offered by these institutions are unparalleled.
That said, there is considerable syllabus overlap between the two exams. Many aspirants prepare for both simultaneously, especially since INI-CET is held before NEET PG. The key is to start with INI-CET’s advanced clinical focus and then adapt to NEET PG’s broader but slightly less conceptual style.
In short, if you're serious about pushing for a top-rank institute, then INI-CET is your mountain to climb. But climbing that mountain will also give you a head start for NEET PG.
