For all the Medical graduate students who complete their MBBS abroad, there is no milestone more significant than qualifying Foreign Medical Graduate Examination or FMGE.
As every year thousands of aspirants return to India with the dream of practicing Medicine here, the competition becomes intense with each passing year. And the difficulty of the exam is typically the reason why many aspirants struggle to cross the FMGE hurdle.
The passing percentage remains low every year. And this is not because students lack knowledge or commitment, but largely because of the gap between what is taught in medical universities abroad and what the FMG exam actually evaluates.

The FMGE exam, however, is structured differently. Conducted by NBEMS, FMGE tests graduates through 300 MCQs across 19 subjects in a single sitting. The focus is on applied knowledge, recall of high-yield facts, Indian public health programs, and image-based or clinically integrated questions.

If you are preparing for FMGE, you are not just expected to cover 19 subjects. You must adapt to an exam style that is often very different from what most foreign medical graduates have experienced during the MBBS. Here are a few most common challenges that students mostly face:
This surely takes the top position in the challenges. Preparing for FMGE requires you to be a master of all 19 subjects of the MBBS curriculum. For most of the students who have studied abroad, many of these 19 subjects might not have been given the same importance. Or there are even chances that certain areas may have been taught differently.
This leaves the students feeling that they are studying some topics for the first time. The pressure to cover everything within a few months between receiving the eligibility certificate and exam day can be pretty overwhelming.
The students preparing for FMGE are sure to be diligent in their approach but the thing that they struggle with is recalling what they learned. The problem is not at all lack of effort but it is lack of revision.
When you have such a vast FMGE syllabus, reading one time simply won’t suffice. Most students make the mistake of moving on to new topics without revisiting old ones. This leads to forgetting much of the earlier content by the exam time.
Most of the aspirants are used to giving descriptive and detailed answers, essay writing or visa-based exams abroad. But when it comes to FMGE, it is usually MCQ-driven. This requires you to be able to apply concepts quickly, eliminate wrong options, and manage time under pressure.
This is undeniably the toughest challenge. Overcoming mental stress while preparing for FMGE is crucial. This exam has a reputation for being uncrackable. And that is typically due to the history of low percentages.
If you keep on consistently hearing stories of repeated failures, you are bound to get anxious even before you start the preparation.
This constant pressure often leads to burnout, inconsistency, and a lack of confidence, which directly impacts exam performance.
Start by analyzing subject-wise weightage. Prioritize high-yield subjects like Medicine, Surgery, PSM, Pharmacology, Pathology, and OBG. Don’t ignore smaller subjects such as Forensic Medicine or Anesthesia—they may carry fewer questions but can make a big difference.
FMGE is all about MCQs. Practice them daily, not only to check knowledge but also to learn exam language, improve speed, and build accuracy. Reviewing explanations for both correct and incorrect answers is essential.
FMGE is not about reading everything once—it’s about revisiting content until it sticks. Follow the 3R Rule:
National health programs (like TB, HIV, polio, maternal health) and Indian treatment protocols are exam favorites. Adding these to your prep gives you an edge.
Mocks simulate the real exam, help manage time, and build stamina for 300 questions in one sitting. Regular mocks also reveal weak areas so you can work on them before the exam.
Success in FMGE doesn’t come overnight. Build daily habits, stick to a timetable, and set realistic targets. Use study groups, topper strategies, or mentorship to stay motivated through the long prep journey.
No one can deny the gap between the foreign MBBS curriculum and the FMGE pattern. But the fact is that it is unbridgeable is what makes it pretty doable.
