Adolescents And Their Development

Biochemical Changes occurring in Adolescents
One of the first neuroendocrine changes at puberty is the maturation of the gonadotropic hormones, or the GnRH pulse generator. Adrenal androgen production happens before testicular androgen production, primarily the dehydroepiandrosterone sulphate (DHEAS). Adrenarche, a disease characterized by symptoms like underarm odor and faint genital hairs, can start as early as age 6.
In the teen years, puberty usually happens a few years following adrenarche. When a youngster achieves sexual maturity, their body goes through changes known as puberty. In contrast to the physical changes that accompany puberty, the main developmental stage of adrenarchy is emotional and psychological. It persists during puberty, with levels of adrenal androgen gradually rising until they reach their peak in maturity.

Menarche: A female adolescent's first menstrual period is known as menarche. The menarche always happens three years after the thelarche.
The onset of the thelarche depends on many factors:
- Genetic factors.
- Environmental factor.
- Weight and nutrition status.

Sexual Maturity Rating/SMR Staging in Females
A woman's pubic hair and breast size are taken into consideration when judging her sexual maturity.
The following table shows the respective sexual maturity ratings based on physical appearance.
SMR Stage Pubic hair Breast 1 None / Preadolescent Preadolescent; elevation of papilla only 2 Sparse, straight, slightly pigmented, straight, at the medial border of labia Breast and papilla elevated as a small mound; areola diameter increased 3 Darker, beginning to curl, increased amount Breast and areola enlarged with no separation of their contours 4 Coarse, curly, abundant, not present on medial thigh Projection of areola and papilla to form secondary mound above the remaining part of breast. 5 Adult-like hair
Also present on medial thigh.Mature breast ,nipples projects, but areola is a part of the general breast contour now.

Developmental characteristics of adolescence in Males
The sequence of pubertal growth in males
In males, the enlargement of the testicles is the first obvious symptom of pubertal growth. This is usually followed by the appearance of pubic hairs, which appear six months later.
Penile development occurs following pubic hair emergence; • The peak growth spurt is larger than in females, but it happens 6 months to 2.5 years later than in a female of the same age.

Sexual Maturity Ratings/SMR Staging in Males
The following table is given below showing the respective sexual maturity ratings and based on the physical appearance
| SMR stage | Pubic hair | Penis | Testes |
| 1 | None | Preadolescent | Preadolescent |
| 2 | Sparse, long, straight ,lightly pigmented. | Minimal enlargement | Enlarged, pink scrotum, altered texture. |
| 3 | Darker, increase in amount, begin to curl | Lengthens | Volume/size increases. |
| 4 | Coarse, curly, Abundant hair ,not present on medial thigh. | Breadth increases, Glans increases in size. | Enlarge further .dark /pigmented scrotum. |
| 5 | Adult like hairAlso present on medial thigh. | Adult size | Adult size |
The following graph represents the sexual development of both males and females.

This graph shows that peak growth velocity in females comes early as compared to males but it is lesser as compared to males.

Somatic Growth Patterns in Adolescence
It happens from distal to proximal, meaning that the body's lateral regions develop more quickly than its central region.
Other Physiological Changes in Adolescence
Modification of lean body mass and fat deposition: Males mostly exhibit an increase in lean muscle, while females primarily exhibit a decrease in fat composition, particularly in the thigh and buttock areas.
Variations in heart rate and blood pressure between males and females are caused by changes in the cardiovascular system. Modifications to the lungs: a rise in vascular capacity. When there are modifications to the globe's elongation, myopia becomes an issue.
Modifications to the larynx and pharynx—far more pronounced in men than in women. Males experience far greater changes in the glandular system than females do. Apocrine and sebaceous gland production is increased.
Modification of the calcium deposit. During puberty, 50–60% of the total calcium is deposited.
