Ectropion: Causes, Symptoms, Risk Factors and Diagnosis

Ectropion is the disease that causes your eyelid to turn outward. As a result, the inner surface of the eyelid is exposed and becomes more sensitive to irritation.

Ectropion often only affects the lower lid and is more prevalent in the elderly. When ectropion is severe, the entire length of the eyelid is turned out. Less severe forms of ectropion cause only a partial sagging of the eyelid away from the eye.

The symptoms of ectropion can be decreased by using artificial tears and lubricating ointments. But to effectively treat the condition, surgery is usually necessary.

Causes Of Ectropion

Ectropion can be brought on by:

• Muscle weakness: As you age, the muscles under your eyes usually weaken and tendons lengthen. These tendons and muscles keep your eyelid taut against your eye. When they weaken, your eyelid may begin to droop.
• Facial paralysis: Certain cancers and disorders, such as Bell's palsy, can paralyse the facial muscles and nerves. Ectropion may arise from facial paralysis affecting the eyelid muscles.
• Scars or previous medical treatments: Your eyelid's position on your eye may alter if you have burns or other traumatising injury to your skin (like a dog bite). Previous blepharoplasty (removal of skin from the eyelid) may cause ectropion, particularly if a large amount of skin was removed.
• Growth of the eyelids: Your eyelid's outward tilt could be the result of benign or malignant growth.
• Hereditary diseases: Rarely is ectropion congenital, which means that it is present from birth. When it is, it is usually associated with genetic disorders such as Down syndrome.

Symptoms Of Ectropion

When you blink, your eyelids typically disperse tears uniformly throughout your eyes, moisturising their surfaces. These tears drain out of microscopic pores located in the punctcta, or inner part of your eyelids.

When you have ectropion, your lower lid moves away from your eye, making it difficult for tears to enter the puncta. The following signs and symptoms could include:

• Watery eyes: Tears may well up and stream down your face constantly if you have inadequate drainage.
• Excessive dryness: Your eyes may feel dry, gritty, and sandpapery as a result of ectropion.
• Irritation: The whites of your eyes and the area around them may become red and feel burning due to irritation caused by dry or stagnant tears.
• Light sensitivity: Dry eyes or tears that don't flow might irritate the surface of the cornea, making you sensitive to light.

Risk Factors  Of Ectropion

The subsequent factors increase the likelihood of developing ectropion:

• Age: The weakening of muscle tissue associated with ageing is the most common cause of ectropion.
• Previous eye surgeries: People who have had eyelid surgery are more likely to develop ectropion later on.
• Previous burns, trauma, or cancer: Ectropion is more likely to occur in those who have had burns, facial injuries, or skin cancerous lesions on their faces.

Diagnosis  Of Ectropion

To diagnose ectropion, a routine physical examination in addition to an eye examination is usually sufficient. Your doctor may ask you to pull on your eyelids or close your eyes tightly during the examination. This helps examine the tone and rigidity of the muscles of each eyelid.

If your ectropion is the consequence of radiation, surgery, a tumour, or scarring, your physician will also examine the surrounding tissue.

Understanding how ectropion is caused by various medical conditions is essential when choosing the right surgical procedure or treatment plan.

Treatment  Of Ectropion

Your doctor may suggest artificial tears and ointments to relieve the symptoms of moderate ectropion. In most cases, surgery is necessary to completely correct ectropion.

Surgery

The state of the tissue around your eyelid and the reason behind your ectropion will determine which type of surgery you need:

• Ectropion brought on by aging-related muscle and ligament relaxation: Your lower eyelid's outer margin may have a minor portion removed by your surgeon. The tendons and muscles of the lid will tighten as it is stitched back together, allowing the lid to rest correctly on the eye. The process is usually not too complicated.
• Ectropion caused by scar tissue following an injury or previous surgery: Your surgeon might need to use a skin graft from your upper lid or from behind your ear to help preserve the lower lid. If you have significant scarring or facial paralysis, you might need a second treatment to entirely correct your ectropion.

Before surgery, a topical anaesthetic will be applied to numb the area around your eyelid. To make you feel more comfortable, you may be given an oral or intravenous mild sedative, depending on the type of treatment you're receiving and whether the procedure is being performed in an outpatient surgical facility.

After surgery, you may need to:

• For the entire day, wear an eye patch.
• Put an antibiotic and steroid ointment on your eye several times a day for a week.
• Use cold compresses regularly to minimise bruising and swelling.

After surgery, you may most likely feel as follows:

• Temporary swelling
• Bruises around your eyes
• You can have eyelid stiffness following surgery. Stitches are usually removed around a week after surgery. The bruises and swelling should go down in about two weeks.

Complications Of Ectropion

Ectropion causes your cornea to become irritated and exposed, which increases the probability of dryness. As a result, corneal ulcers and abrasions could be a risk to your vision.
